Specific fees will be discussed with you at the onset of services. I currently accept cash and checks. Sliding scale rates are available for individuals who can demonstrate financial hardship. Please contact me for more information.
Insurance
I am in an-network provider with Blue Cross Blue Shield of Texas, Aetna, Seton Health Plan, and Humana. If you are a member of Blue Cross Blue Shield, Aetna, Seton Health Plan, or Humana and would like to utilize them for billing, please provide me with your insurance information prior to our initial meeting so I can obtain pre-authorization for services. Typically, I will collect a co-payment from you at the time of service. The co-pay amount is determined by your insurance company and varies dependent on your policy.
If you are not a member with Blue Cross Blue Shield, Aetna, Seton Health Plan, or Humana and choose to pay out-of-pocket, your insurance company may reimburse you for a percentage of my fee. I will provide you with a coded receipt to submit to your insurance company as a reimbursement form, but your coverage may have limitations for out-of-network providers.

Cancellations
If you must cancel a session, please notify me at least 24 hours prior to your scheduled time. If I do not receive notification, there will be a previously agreed upon fee, as I have reserved the time for you. The no-show fee may be waived for emergency situations.
